Need to make a payment? Visit our pay my bill portal page, where you can complete your payment quickly and securely. If you have any questions or need assistance with the payment process, please don’t hesitate to reach out to our support team.
Authorization of health information form
A Privacy Rule Authorization form is a formal consent given by an individual that allows a covered entity to use or share their protected health information (PHI) as specified in the Authorization form. This consent outlines the specific purpose and recipients for which the PHI can be utilized. SCRI Oncology Partners must receive informed consent from patients before they can be involved in research. You can download the form below or request a copy at our front desk.
Request medical records
To get a copy of your medical records from SCRI Oncology Partners for yourself or someone else, you must complete a Medical Records Request form. You can download the form below, or complete it at the SCRI Oncology Partners front desk at your next visit. There is no charge for records sent to you or your physician although there may be a charge for records sent to insurance companies and other third parties. A request for medical records will be responded to in no more than 15 working days from the receipt of the request, with records needed for emergency care processed in a more timely manner.
Please let us know the format you prefer for your medical records, whether that is paper, fax, encrypted email or CD.
